Pathogenesis and Course of AIDS Title: Give a detailed account of the pathogenesis and course of AIDS. Undergraduate Degree Level Essay 2,500 words   The study of HIV / AIDS is a vast topic and the literature on the subject fills many volumes. In this essay therefore we propose to take an overview of some of the most current views and developments in the field with particular emphasis on the pathophysiology of HIV / AIDS In 1997 the World Health Organisation gave the assessment that since HIV / AIDS had been recognised, over 11.7 million people had died of the condition world wide and at the time of publication 30 million more were thought to be infected with 16.000 new infections occurring daily. Current predictions estimate that at the current rate of infection 55 million will have died by 2010. (Greek R et al 2002) Perhaps the most worrying of all of these gargantuan statistics was the fact that of the 30 million infected, 27 million were thought to be unaware of their condition. Quite apart form the devastation the disease causes on a personal basis, the vast majority of those infected are young adults which has enormous implications for the social structure of their communities. (Graham B S 1998) Pathophysiology of the condition As we have implied earlier, the volume of work relating to the pathophysiology of HIV / AIDS is enormous, in this essay we therefore intend to â€Å"cherry-pick† a number of selected topics and discuss them in some detail. The implications of genetics in both the acquisition of HIV and the subsequent development of AIDS is a rapidly expanding field. The interaction between virus and host is a multifaceted and extremely complex one. From the point of infection onwards there is usually a significant HIV viraemia even though in the early stages, the patient may be completely asymptomatic. It is known that the degree of virus replication is directly related to the degree of T-cell depletion and equally correlates with progression of the disease process. It would therefore appear that HIV induces symptomatic disease process by replicating in, and subsequently destroying, CD4 and T-cells thereby weakening the immune system. (Stilianakis NI et al 1997), .Different hosts and indeed different genotypes of hosts (see on) have differing patterns of disease expression. CD4 and T-cell levels are rapidly diminished in the early stages of the disease but are not restored by effective anti-viral therapy if given later in the disease. (Littman D R 1998) One area of obvious interest is in those who appear to survive with HIV for a longer than average time before it progresses to AIDS. A study by Dean (M et al 1995) proved to be seminal in this area, with a prospective study of nearly 2,000 men. The authors considered the status of CCR5 genotype and its relation to the likelihood of disease progression. The paper is both long an detailed, but provides a strong evidence base for further research (Berwick D 2005). In essence, the main findings of the paper were that most people have two normal alleles for the CCR5 gene, but 1 in 7 has one mutant allele (technically 32bp deletion), which means that they still have one normal allele (heterozygous genotype). 1 in 100 have two mutant alleles. The rates of mutation are highly racially specific ranging from 11% in Caucasians to The significant finding in the study was that none of the 1,300 HIV +ve people in the study had the homozygous mutation, 15% of the HIV +ve had the heterozygous genotype, so the heterozygous genotype clearly does not protect against infection, but the significant difference is that the average transition time from HIV to AIDS for the homozygous man was 10 years whereas the average transition time for the heterozygous genotype was 13 years. Possibly even more significant is the fact that of the 17 people in the entry cohort who were homozygous for the mutation and in the high risk of infection group, none of them had contracted HIV. It would therefore appear that the CCR5 mutation plays some critical role early in the primary stages of HIV infection since it appears that HIV infection can be blocked if a functioning version of this receptor is not present. During the later stages of the infection it would appear that other co-receptors (the CXCR4 has been implicated) can take over the role as the properties of the virus evolve within the host. (McMichael A 1998). On this basis some authors have suggested a classification taxonomy that differentiates HIV virus sub-types on the basis of their CCR5 receptor affinity. (Berger E A et al 1998). It would appear that the viruses eventually evolve into the R5X4 (in this classification) type which allows them to eventually produce the full blown AIDS syndrome. The absence of one working CCR5 allele simply retards the evolutionary progress. (Chan DC et al 1998), This is in congruity with other pathophysiological observations. For example, it is already known that the influenza virus enhances the CXCR4 dependent HIV infection. It is thought that the pathway of influenza infection activates the CD4 and T- lymphocytes which, in turn utilise the CXCR4 co-receptors on the cell. This activation would therefore appear to increase the potential number of HIV target cells in an individual which would clearly accelerate viral spreading. (AIDS RU 1998). In the same way, syphilis is known to be an active agent in increasing CCR5 expression and is also known to be a strong predisposing factor for the overall HIV risk whereas it does not induce CXCR4 (Lafeuillade A et al 1997), From our considerations thus far it is clear that the pathophysiology of the HIV infection revolves around the build up (replication) of the HIV virus in the CD4 and T-cells. This is not an immediate process as new T-cells are being produced (albeit from a progressively dwindling stock) of non-infected bone marrow stem cells. (Greek R et al 2002) Why are there a number of specific AIDS-defining diseases? This is a vast area in its own right. The presence of HIV in a T-cell does not immediately destroy the cell, but alters its function. Each T cell has a number of receptor areas determined by the V region of the receptor gene, and these determine the subclass (and specificity) of the T-cell itself . Each sub-type has specific receptor sequences that allow it to recognise a broad spectrum of histocompatibility complexes. (Hecht F M et al 1998) The HIV presence alters the expression of the V site region and thereby allows certain pathogens to be sub-optimally challenged (Connors M et al 1997). It is the nature of HIV infection that specific colonies (or sub-types) of CD4 T-cells are depleted before others are altered. This translates clinically into the situation where certain pathogens ( viz. Pneumocystis carinii, Mycobacterium avium-intracellulare, and cytomegalovirus. ) can be present, virtually unchallenged even though the T-cell population may be apparently quite active. Typically the reservoir of CD4 and CD8 lymphocytes may remain skewed despite the overall apparent adequacy of circulating T-cells. (Nosik M N et al 2002), Alongside this altered state of immunity a number of other immune-related phenomenon can be seen including some types of autoimmunity and AIDS-related malignancies including squamous cell carcinoma of skin, testicular cancer, myeloma, Hodgkins disease. Some investigators have recently demonstrated a statistically very significant relationship between a profound immunodeficiency state (with marked CD4 depletion) and the development of a non-Hodgkins lymphoma, presumably by a similar mechanism. (Voulgaropoulou et al. 1999) Aggressive anti-viral therapy has been partially successful in reducing the frequency of malignancies such as Kaposis sarcoma and B cell lymphomas. Study of these progressive â€Å"blind spots† in the T-cell’s response mechanisms suggest that a diversity of the T-cell receptor V genes can be re-established in patients with an undetectable viraemia for longer than a six month period, which is strongly suggestive of the fact that regeneration of uninfected (or immuno-protected) naà ¯ve precursors is possible with aggressive therapy. (Connors M et al 1997), There is an overall increase in the incidence of AIDS-related malignancies. This is not thought to be due to any new or progressive evolution of the HIV virus, but mainly due to the development of new and more effective antiretroviral therapies together with more efficient prophylaxis for opportunistic infections which is allowing the HIV / AIDS patient to survive for longer in the immunodeficient state. Treatment We do not intend to present any detail relating to specific treatments for HIV / AIDS but will make a few general comments. A current pressing question for clinicians is â€Å"can antiretroviral therapy ever be safely stopped?† The current generations of protease inhibitors that are combined with non-nucleoside reverse transcriptase inhibitors are capable of reducing viraemia to undetectable levels. (Jordan R et al 2002), Clinical experience suggests that as soon as treatment is stopped, viraemia tends to rapidly recur at pre-treatment levels. This strongly suggests an ability of the HIV to enter a latent phase or to remain in immunoprivilleged sites (such as the testes and central nervous system). Like most retro-viruses, the HIV has the ability to integrate its DNA into the host genome even though it may remain transcriptionally dormant and thereby avoid cellular detection and apoptosis until it enters its replication cycle (Wei X et al 1995), It is difficult to draw specific conclusions from a presentation such as this as the overriding impression that one gets from any examination of the literature on the subject is both the speed and the diversity of the research that is currently being undertaken world-wide. There appear to be two main thrusts as far as research is concerned. One is the development of new antiretroviral and immunoactive therapeutic measures to try to combat the pathophysiology of the disease process itself, the other is the search for a vaccine which would ultimately be the â€Å"holy grail† in this particular pandemic. (Malegapuru W et al 2002) One of the main stumbling blocks as far as vaccine development is concerned is the difficulty in targeting the antigenicity of the frequently changing immunological profile of the HIV. (Musey L et al 1997). Considerable interest has been shown in the persistently sero-negative partners of sero-positive patients who have been frequently found to have a specific ability to produce interleukin 2 from peripheral mononuclear cells together with the detectable presence of HIV specific IgA in mucosal secretions. (Mazzoli S et al 1997), Many vaccine research projects are currently exploring the avenue of designing vaccines which have the potential to stimulate and produce HIV-specific CD8 cytotoxic T-cell responses to the HIV. Initial primate studies suggest that prevention of infection at a mucosal site (as opposed to parenteral infection) is actually possible as mucosal infection is relatively inefficient and only a small number of HIV virons are likely to be involved. (Matano T et al 1998). Phase one clinical trials have been undertaken in this regard already but with disappointing results as the immunogenic responses that have been engendered are 5-10 times lower than those produced by HIV infection with a comparatively short half-life. (Mugerwa R D et al 2002). There are a number of approaches with recombinant viral entities of various types which have also met with limited success Currently it would appear that vaccine candidates can manage to induce CD8 cytotoxic T lymphocyte responses with killing activity across different strains which can last a significant length of time, but they are yet unable to induce neutralising antibody with activity against typical transmitted HIV virus. Pros and Cons of Inclusive Education Essay

In inclusive education programs all students in a school are subjected to study together and become part of the school community regardless they are disabled or normal (Schwartz, Odom, & Sandall, 2010). They all attend the same school and have the feeling of belonging in the school. Students, teachers and co-ordinate staff have a role to instruct all students regardless to their disability in general education. From educational viewpoint, inclusion involves practices where students with special needs in terms of education spend much of their time with normal student in the learning process. It is believed that inclusive education poses more merits than its negatives effects to educational systems since it provides a better opportunity for all students to participate in all activities in their everyday learning process. Inclusive education has been a platform for the disabled students to create a unique and better relationship with other normal students in school communities. Additionally, inclusive education serves as a fundamental right to all disabled students since they are part of the society and thus they should not be ignored (Schwartz, Odom, & Sandall, 2010). While there are a number of controversies in the subject of inclusive education, a clear understanding is required about inclusive education so as to reap maximally from the benefits it presents. Advantages of Inclusive Education The current research seems to support inclusive education and co-teaching as means of improving both special and general education in the schools (Block, n. d). Mainstreaming which is typical to inclusive education has been observed to improve students’ social skills by fostering interaction between the disabled students with other normal students in the same or neighboring schools (Block, n. d). This has developed communication and interaction skills among the normal and disabled students thus making them to be competent in the society. In particular, the increased interaction among students in inclusive education develops them to relate well with other members of the society. Social dimension is measured in terms of behavior and character of an individual in connection with the capacity to exercise them. Communication forms the basis of character and behavior of individuals. Therefore, inclusive education builds individuals who perfectly fit into the society (Block, n. d). Higher self-esteem among disabled students has also been achieved in the process of mainstreaming typical in inclusive educational system (PBSparents, 2010). Students gain more confidence and self efficiency in inclusive education hence they command a higher self-esteem. Not only do students in inclusive education gain higher states of confidence, they also gain systematic ways to approach common aspects in life. Research conducted in California on disabled students showed that 96 percent of disabled students felt confident when mixed with other students during learning process (PBSparents, 2010). However, 3 percent of these students felt excluded while a lesser percentage of 1 percent felt low even when mixed with other normal students (PBSparents, 2010). This research shows that inclusive education builds high esteem among the disabled students and hence should be encouraged (PBSparents, 2010). Enhanced academic achievements among the disabled students form another great advantage of inclusive education. Through the integration of normal students and the disabled, there is a potential realization of academic achievements among the disabled students who are a minor group in the society (PBSparents, 2010). The relationship between the teachers and students is much coherent in inclusive education and thus creates smooth means for academic achievements among this minor group. Most teachers take keen interest to the disabled students in order to put them in the same level with the rest of other in the class. Students gain both academic knowledge and applied abilities that are critical in their lives. As a result of inclusive education, there have been increasing numbers of disabled graduates in the US and elsewhere in the world (PBSparents, 2010). In inclusive education, there is an enhanced parents’ participation in the provision of education to the students (PBSparents, 2010). This enhanced participation ensures that students are provided with better education hence better academic achievements on the part of students. This is achieved through moral and material support that parents invest in the students which lead to improvement in education quality in inclusive education. The learning objectives are well met in inclusive education as parents commit their efforts in the betterment of education (PBSparents, 2010). As a result, there is an improved relationship in inclusive education compared to exclusive education system where parents rely entirely on teachers and the school management on education responsibilities for their children (PBSparents, 2010). Normal students in inclusive education learn to appreciate other disabled students hence fostering better relationships not only in the school community but also at their homes and the society at large (Ratta, 2009). Inclusive education brings out the meaning of the theoretical knowledge taught in schools into practice. The majority of the learners in other education systems that do not mix normal students with disabled students leave schools with limited knowledge thus making it hard for them to cope up with life (Inclusive Schools Network, 2010). It is believed that when the normal students learn in the same environment with the disabled students, much understanding and tolerance is cultivated . the normal students learn to accept others in their capacity regardless to their health (Ratta, 2009). Positive aspects achieved from inclusive education contribute much to contact theory which asserts that meaningful, frequent and pleasant communication between and among people results to a better and changed attitude towards differences. As result much is felt in the society of different individuals. On the same note, inclusive education prepares the disabled students to plan on their future (PBSparents, 2010). The lessons they achieve in inclusive educational programs while in schools are crucial in preparing them for future career and general tactics of survival. In inclusive institutions of learning, teachers impart knowledge to learners. This knowledge usually consists of theory as well as practical skills. This ways, students become more flexible in life and have some ways to survive. Life is much of what one knows and gets experienced to do (PBSparents, 2010). Therefore, there is always an increasing need for inclusive education that combines both the disabled and non-disabled students. More so, inclusive education helps disabled student to develop a sense of pride in their learning due to accomplishing their duties in the process of learning. For instance, under normal class environment with various students (normal and disabled), given an assignment to do will increase the pride of disabled learners if accomplished in time and accurately (PBSparents, 2010). This is clear indication that learning inclusively improves the ability of the students in general. Demerits of Inclusive Education Not only does inclusive education provide the best to both normal and disabled students but also constitutes of a number of disadvantages (Schwartz, Billingsley & McBride, 2010). One of the most debated disadvantages of inclusive education is financial hardships. In order for the school to provide for special education system and services, much financial resources are needed to properly finance the system. The cost per student under special education is much higher compared to normal student thus giving a challenge to most schools. The more special students a school has, the more expenditure needed to run the school (Schwartz, Billingsley & McBride, 2010). This has caused more problems in inclusive education system. Parents and guardians have often expressed fears that in inclusive education system, teachers do not have the required skills to teach and accommodate the disabled learners (Schwartz, Billingsley & McBride, 2010). However, supportive and training institutions usually train the teachers on both normal and special needs of students so as to address problems in case they arise. But if the teacher ignores the students of this nature it may result to regression of the learner thus the overall productivity is decreased in terms of academics. More so, the learners them selves may cause harm to the fellow students thus making classroom environment uncomfortable for learning process (Schwartz, Billingsley & McBride, 2010). One serious and potential disadvantages of inclusive education is that of the amount of time and the kind of attention drawn by the teachers to students with special needs (Kidstogether. org, 2010). Significant observations have shown that there is an increased attention to students with special needs which may take away the attention of the teachers from the rest of the class. This trend can eventually lead to a decreased concentration on the part of normal students hence less achievement academically (Kidstogether. org, 2010). In another observation, due to frequent disturbances in inclusive education, there may be a fall in academic performance (Schwartz, Billingsley & McBride, 2010). In most cases, it’s believed that learners with disabilities can cause unnecessary locomotion in learning environment making it tireless exercise. Teachers may lack positive and dynamic response to the students due to lack of special skills to handle such learners in classroom environment. This is much contributed by personal view towards the disabled people. Lack of official liaison required by the teacher can also interrupt the usual understanding of the learner in this perspective (Schwartz, Billingsley & McBride, 2010). Improper articulation between the co-teachers also contributes to students in taking, no matter their abnormality. Another major issue is social discrimination among the teachers and students. This will cause serious problems not only in the school environment but also in the community level. Regular education students may so much empowered to do anything to disabled individuals this resulting to inferiority complex among the students. This can be avoided by having a joint socialization in the school which may extend further to the outside communities (Schwartz, Billingsley & McBride, 2010). Studies have shown that students with special needs may require more time to grasp concepts (Kidstogether. org, 2010). Time spent in reviewing concepts may not be enough for students with special needs who learn at a slower pace. Normal students who need to move at a fast pace may end up getting bored and if this weakness is not checked, it will dangerously lead to poor academic performance among normal students in inclusive education. This is because inclusive education system overlooks the needs and requirements of the both normal and disabled students (Schwartz, Billingsley & McBride, 2010). The rise in implementations and modifications usually articulated in regular education systems that practice inclusion has caused disconnection between teaching and the learning process (National Center for Education Statistics, 2010). This has mainly affected special need students in these systems which practice inclusion. When rules are further implemented, it becomes hard for disabled students to adhere to and incase no adjustments are made, this can ultimately affect the standards of learning among the special need students. The results may be dwarfism in education either in terms of skills or knowledge or both (National Center for Education Statistics, 2010). Most researchers have given out different views concerning the idea on inclusive education system where other have observed that the system is so much demanding to school administration and the community at large (National Center for Education Statistics, 2010). Other researchers have observed an increase in school closure in the systems that embrace inclusion. This presents as one of the challenges of inclusive education system which has lately been perceived to be difficult to manage and run making many schools to shut down and students’ school dropouts (Delma, Selina & Dorothy, 2009). A significant amount of resources is dedicated to field work and mainstreaming where normal students benefit more compared to rest of the disabled students. This presents a serious inequality in per student spending where resources are spent for functions that do not benefit the students with special needs (Kitmitto & Bandeira, 2008). However, this point has been argued by a number of observers who note that special needs students also require facilities and equipments not required by normal students. This observation seems to cancel out the point that more funds are spent on normal students. This controversy has resulted to the formation of ripples in a legal system of education (Kitmitto & Bandeira, 2008). More serious problem is when some groups believes that some students are not worth to be put in regular classroom to study thus resulting to streaming in school communities. Conclusion In conclusion, inclusive education system is slowly becoming forgotten in most schools despite its importance among both normal and disabled students. It has many several benefits as opposed to other educational system which do not combine disabled students with the normal ones. It should be identified that the ability of the disabled students to socialize with their fellow students to a large extent build their confidence as the student feel to be part of the school community. Attempts to prevent the students to access inclusive education prevent the students from a number of benefits that would otherwise mold them to become successful leaders. Therefore, parents, teacher and all school stakeholders need to adjust in order to accommodate both disabled and normal students in the same learning environment bearing in mind that they are all part of the community. In what follows, these two terms are used interchangeably. The forbidden of riba is mentioned in four different revelations in the Quran. The first revelation highlights that interest removes wealth of Gods blessings. The second revelation condemns it, placing interest in combination with illegal appropriation of property belonging to others. The third revelation enjoins Muslims to keep on clear of interest for the sake of their own welfare. The fourth revelation set up a clear difference between interest and trade, influencing Muslims to take only the principal sum and to forgo even this sum if the borrower is not capable to repay. It is further declared in the Quran that those who ignored the forbidden of interest are at war with God and His Prophet. The forbidden of interest is furthermore cited in no unsure terms in the Hadith (sayings of the Prophet). The Prophet warned not only those who take interest but also those who offer interest and those who record or witness the operation, saying that they are all similar in guilt. It may be mentioned in passing that similar prohibition are to be found in the pre-Quranic scriptures, although the People of the Book, as the Quran refers to them, had chosen to rationalize them. It is remarkable that Islam has successfully warded off various subsequent rationalization efforts aimed at legitimizing the institution of interest. Some scholars relays on economic reasons to explain the reasons on why interest is prohibited in Islam. scholars argued that interest is a pre- determined cost of production, which avoid full employment (Khan l968; Ahmad n.d.; Mannan l970). In the same tone, it has been challenged that international monetary crises are largely due to the institution of interest (Khan, n.d), and that trade cycles are in no small measure attributable to the phenomenon of interest (Ahmad l952; Suud n.d.). None of these studies, however, has really succeeded in creating a causal link between interest, on the one hand, and employment and trade cycles, on the other. Others, anxious to maintain the Islamic position on interest, have argued that interest is not very effective as a monetary policy instrument even in capitalist economies and have questioned the efficiency of the rate of interest as a determinant of saving and investment (Ariff l982). A general line running through all these negotiations is the exploitative character of the institution of interest, although some have pointed out that profit (which is legalized in Islam) can also be exploitative. One response to this is that one must differentiate between profit and profiteering, and Islam has prohibited the latter as well. It began as a theological dream, but Islamic banking has become a practical reality across the Middle East. The question now is, how far will Sharia boards and western regulators let it spread? (Josh Martin, Middle East, Jun2005 Issue 357, p50, 6p) The Islamic prohibition on interest does not imply that capital is costless in an Islamic system. Islam realizes that money is a factor of production however; it does not permit the factor to pre-determined claim on the productive surplus in the form of interest. This has leaded to the question as to what will then substitute the interest rate mechanism in an Islamic framework. There have been propositions that profit sharing can be a viable alternative (Kahf l982a and l982b). In Islam, the owner of capital can officially share the profits made by the entrepreneur. What makes profit- sharing allowable in Islam, while interest is not, is that in the case of the former it is only the profit-sharing percentage, not the rate of return itself that is predetermined. It has been argued that profit-sharing can help assign resources efficiently, as the profit-sharing percentage can be prejudiced by market forces so that money will flow into those sectors that offer the highest percentage of profit-sharing to the investor, other things being equal (Naqvi, l982). However, ordinary Islamic thinking on this subject obviously points to the need to replace interest with something else, although there is no clear agreement on what form the alternative to the interest rate mechanism should take. The issue is not determined and the search for an alternative persists, but it has not detracted from efforts to experiment with Islamic banking without interest. So according to these advantages and inducement, Islamic banking and finance is no longer designed to serve the needs of the Muslim society only. The industry has started benchmarking its products to conventional suppliers in terms of consumer convenience, benefits, pricing, transparency of terms and conditions, and service standards. Consequently Islamic banking and finance products have expanded popularity with non-Muslim societies due to their competitiveness and efficiency. Recently Lloyds TSB Bank in the United Kingdom began offering its individual customers Sharia-compliant current accounts and home finance products. 2.4 PRINCIPLES AND CONCEPTS IN ISLAMIC BANKING Islamic banking has the same principle as conventional banking excluding that it works in accordance with the rules of Syariah, known as Fiqh al-Muamalat (Islamic rules on transactions). The fundamental principle of Islamic banking is the distribution of profit and loss and the forbidden of riba (interest). This section defines various concepts related to Islamic banking: Wadiah (Safekeeping): In Wadiah, a bank regarded as worth keeping of the funds. An individual deposits his money in the bank with bank guarantees to refund of the whole money that was deposit when the person demands for it. The depositor may be rewarded with hibah (gift) as a bank appreciation for him for using his funds to finance their activities (Ali, R. 2008). Mudharabah (Profit Sharing): which is an agreement between a person or a bank who have capital and a businessperson, whereby the businessperson can move funds for its business activity. The profits generated from the business will be shared between the two parties depending on the agreed between, however losses are borne solely by the capital provider (Ali, R. 2008). Musyarakah (Joint Venture): which is business partnerships or joint ventures where the profits made are shared on an agreed ratio while losses incurred, will be divided based on the equity participation ratio. Murabahah (Cost Plus): which mean selling of goods at a price includes a profit margin agreed by both parties. The purchase and selling price, other costs and the profit margin must be clearly stated at the time of the sale agreement (Ali, R. 2008). Bai Bithaman Ajil (Deferred Payment Sale): which is selling of goods on a deferred payment basis at a price includes a profit margin agreed by both parties(Ali, R. 2008). Wakalah (Agency), which mean that when a person appoints a representative to undertake transactions on his/their behalf (Ali, R. 2008). Qardhul Hassan (Benevolent Loan), which is a loan that extended on a goodwill basis while the borrower person is only required to repay the amount borrowed. However, the borrower may, at his discretion, pay extra (without promising it) as a token of appreciation (Ali, R. 2008). Bai al-Inah (Sell and Buy Back Agreement): The financier sells an asset to the customer on a deferred payment and then the financier for cash at a discount (Ali, R. 2008) immediately repurchases the asset. Hibah (Gift): A gift or a given voluntarily in return for loan given or benefit obtained (Ali, R. 2008). 2.4 ISLAMIC LAWS ON TRADING The holey Quran has prohibited gambling, similarly the hadith literature, in addition to prohibiting gambling has also prohibits bayu al-gharar (trading in risk, where the Arabic word gharar is taken to mean, Risk). The Hanafi school in Islam has defines gharar as that whose consequences are hidden, while the Shafi school defines it as that whose nature and consequences are hidden, similarly, the Hanbali school defines gharar as that whose consequences are unknown or that which is undeliverable, whether it exists or not.. Finally, Ibn Hazm of the Zahiri school defines gharar as, gharar is where the buyer does not know what he bought, or the seller does not know what he sold. Islamic scholar has clarifies that gharar is the sale of probable product whose existence or features are not certain, for the reason that risky nature that makes the trade same as gambling (Iqbal, Zamir Abbas Mirakhor, 2007). There are a number of hadith inhibt trading in gharar, often giving particular examples of gharhar dealings (e.g. selling the birds in the sky or the fish in the water, the sperm and/or unfertilized eggs of camels, the catch of the diver, an unborn calf in its mothers womb, etc.). in their book, Iqbal, Zamir Abbas Mirakhor (2007) explains that jurists have looked for many complete definitions of the term and they started with the concept of yasir (minor risk); a financial trading with a minor risk is considered to be allowable while trading in non-minor risk (bayu al-ghasar) is considered to be banned and prohibited. Gharar was never fully determined upon by the Muslim jurists because of the difficulty in deciding what is and is not a minor risk. (Iqbal, Zamir Abbas Mirakhor, 2007) Recently, derivatives tools (such as stock options) have only become familiar relatively. Some Islamic banks offer brokerage services for stock trading and perhaps even for derivatives trading (Iqbal, Zamir Abbas Mirakhor, 2007). 2.6 CONCLUSION Islamic banking is in a rapidly growing stage as a feasible alternative to conventional banking. Although innovative solutions for some issues are still lacking, it would be unfair to compare its progress to that of established conventional finance. There will be no justification in looking for disadvantages in it in contrast to the problems of conventional banking today, especially in the light of the current crisis in South East Asia, where countries had to call out for a revamping of the conventional financial system. It is important to keep the spirit and flame growing in the progress of Islamic banking. In fact, developments in Islamic finance are of great interest to us worldwide. As with conventional financing, Islamic banking benefits from their transparency, good governance and an internationally accepted regulatory framework that governed this important form of financing especially when the decision makers who govern the Islamic banking process manage to find the way for more competitiveness under the Sharia umbrella. Therefore, it is important for the Islamic banking industry to keep working with regulators in the West to make Sharia-compliant products more available in the West, but much of the demand for these products will continue to come from the Muslim world. The Islamic banking industry can evolve into being more sophisticated and modern only if promoters of Sharia technological organizations and professional advisers work together to meet the regulatory, legal and judicial challenges resulting from operating in jurisdictions which implement differing legal systems and compliance policies. Also, to overcome the main obstacles that slow the expansion of Islamic banking world-wide and to meet the worldwide demand for advanced and sophisticated Islamic products, the industry and its advisers will need to continue to work closely with many different regulators and developers to create legal structures within which to operate in a secure Islamic banking manner. CHAPTER THREE METHODOLOGY In this chapter, the study will present the methodology for the present study. The chapter will be divide into four sections; the first section presents the questions of the study, the second section presents the data collection instruments, the third section will presents the data collection procedures, and the final section presents data analysis. 3.1 RESEARCH DESIGN Studies suggest that a study should have a clear plan to answer the research questions (Saunders et al., 2000). Similarly, Yin (2003) explained that a research design is the logic of collecting data to specifically answer a research question. This logic should thus support the researchers to empirically test the presented questions and hypotheses (Yin, 2003; Mouton, 2002). Studies explain that selecting a research design is considered a very important decision of a study (Creswell, 2003). Moreover, studies suggested that there are two types of research designs to select from when conducting a study: quantitative or qualitative design (Hair et al., 2006). Qualitative and Quantitative approaches are often used in social science research studies (Zikmund, 2003; Churchill et al., 2002). These two research designs are dissimilar in using and the choice of a research design depends on the research problem (Hair et al., 2003). The qualitative design suggests examining the variables in the natural context where they can be found. For the qualitative approach, the interaction between variables is essential. Qualitative research involves the non-numerical examination and interpretation of observation for the purpose of explaining the underlying phenomena (Zikmund, 2003). Qualitative design collects data through open-ended questions that provide direct quotations, while the interview is the main method of the investigation (Yin, 2003). The researcher is the main instrument of the study, while the data is collected through interviews (Yin, 2003; Zikmund, 2003). Studies suggest that qualitative design reinforce the process and meaning that are not deliberated in terms of quantity, amount of intensity, or frequency (Zikmund, 2003). The quantitative research design stresses on examining the relationship between one thing (independent variable) and another (dependent variable) in the selected sample. Data gathered through survey provides numerical evidence that should answer the hypothesis or the questions of the study (Zikmund, 2003). Quantitative approach is deemed as the most effective way to address research hypothesis. Qualitative approach is used when the study considers a great closeness to the respondents, their usage and perspectives (yin, 2003). Qualitative approach is more flexible and largely depends on the researchers explanations, reactions and emotions (Yin, 2003). The current study carries out many questions that need a more flexible research design. Due to the nature of this study and the hypotheses presented, the current study uses quantitative data. As main instruments, the study uses questionnaires to collect numerical data. Furthermore, the researcher believed it important to clarify that the current study is applied research, as the outcomes and recommendations may assist Islamic banks in Kuwait to understand customer perceptions toward various issues related to Islamic Banking. 3.2 RESEARCH QUESTIONS To achieve this purpose, the present study seeks to provide empirically based answers the following research questions: What is the degree of customer awareness toward Islamic accounting theory? What are the current efforts of Islamic banks in improving customer awareness toward their practice and theories? What are the most common Islamic services that customers utilize when choosing Islamic banks? What are the customers attitudes and perception toward Islamic banks? What are reasons behind dealing with Islamic banks? The scales used in previous studies provided the basis for developing the research questionnaire. Due to availability issue of the previous study in this field, the study will go through tests to check the validity and reliability of the survey and its procedures. In order to make sure the used survey is valid, the questioners will be send to 20 potential respondents and then evaluate their responses. To find out wither the survey is reliable or not we will measures the internal consistency which is the most popular methods of estimating reliability. The questioners will be arbitrarily split into two groups and then measure the correlation between the two subsets of questioners. For a better result we will use statistic known as Cronbachs alpha which is based on the mean (absolute value) inter-item correlation related to all possible variable pairs. Cronbachs alpha provides a conservative estimate of reliability, and commonly represents the lower bound to the reliability of a scale o f items (Zikmund, 2003). 3.3 INSTRUMENT DEVELOPMENT To answer the aforementioned research questions, a questionnaire in English was designed with a set of questions that were mainly developed from the literature especially the efforts of Kamal Naser (1999) in measuring the degree of customers toward Islamic services in Jordan. The data collection instrument is a 3-page self-administered questionnaire, which consists of two sections (see Appendix A). Section 1,2,3 and 4 was put in a way to collect information on customers perception and attitude toward Islamic banks. Using a 5-point Likert-type scale, respondents rated their agreement from (1) Strongly Disagree to (5) Strongly Agree. Each of these dimension were adopted from different study that was presumed to be relevant to this study. Customers perceptions were deemed to be very high if the mean (average) value was between 4 and 5. However, if the mean value was between 1 and 2, they were deemed to have difficulty to move across future or next generations. A mean value that equals 3 was considered to have average (moderate). 3.4 RELIABILITY AND VALIDITY Previous studies suggest that it is important to check and measure the validity and the reliability of the questionnaire. Zikmund (2003) suggests that validity emphasizes on whether the findings of the study are really about what they appear to be about. He stressed that validity is a significant element for any research project because it is concerned with whether the findings are really about what we intend to measure. While, on the other hand, Reliability defined as; the degree of which a study generates the same result on different occasions and by different researchers (Saunders et al., 2000). The questionnaire items of this study were designed and implemented in prior researches and studies. However, to eliminate the possibility to getting wrong answers, a study should give more intention to validity issues (Saunders and Thornhill, 2003). The following chapter discusses and summarizes the validity and reliability of the questionnaire. 3.5 PILOT STUDY It is important that a study checks the validity and reliability of the data collection instruments before the distribution process. Thus, a pilot test will be conducted by distributing the questionnaires to Islamic banks customers. A total of 15 copies expected to distributed as a pilot test. To test the validity and reliability, SPSS (Statistical Package for Social Science) version 19 and Cronbachs alpha test will be employed. Crombachs Alpha is a common statistical measure that is used for estimating the reliability of indicators. Methodologist suggested that the closer Cronbachs alpha is to 1.0, the more reliable is the scale (Zikmund, 2003). Accordingly, Crombachs Alpha computed and the results compared achieved the recommended cut-off-point (0.7), which indicated that scales were reliable (Zikmund, 2003). 3.6 SAMPLING DESIGN Using random and convenience approaches, the study will distribute around 150 questionnaires among Islamic banks customers who come to the distribution location at bank complex in Kuwait city, where all Islamic banks are located there. The study will distribute the questionnaire by using both email and hardcopy approaches. Using his personal relationships and connections, the study attempted to enhance to respondents rate. The study assured participants that their personal information and responses would be used only for research purposes and would be kept strictly confidential. 3.7DATA ANALYSIS In order to analyze the collected data, the study incorporate Statistical Package For Social Science (SPSS) version 19 to analyze the results of the questionnaire. First, all the negatively worded statements were reversed coded before any statistical analysis was done. The study implements various statistical tools such as: Descriptive analysis (Logarithm Mean, Frequency, and Standard Deviation) was conducted. Descriptive statistical tools employed to describe the participants personal characteristics and their general profiles. Chronbachs alpha test. Zikmud (2003) suggests that Chronbachs alpha value can be used to describe the reliability of factors extracted from multi-point formatted scales. He suggests that Alpha value of 0.70 is deemed a desirable threshold, while a value of 0.60 is acceptable as the minimum cut-off point. To test the relationship between the variables of the study, the Personal Correlation test will be implemented. 3.8 SUMMARY In summary, the chapter presented the methodology adapted in this study. The purpose of this study is explanatory in nature, while descriptive purposes were implemented to assist the primary research purpose. The study uses quantitative research design. The data collocation instruments were developed from previous literature and studies. Prior to the questionnaire distribution, a pilot study was implemented to make sure of the validity and reliability of the questionnaire. The sample of the study is Islamic banks customers in Kuwait who visit the interval location. The study employed various statistical tools to provide empirical evidence that would answer research questions and either support or reject the research hypotheses.
